I finally got gutsy enough to delete everything in my Trash and distfiles folders on my Gentoo box. I've had too many bad experiences deleting things, so I'm pretty gun-shy.
Here's the skinny on keeping your disk...well, skinny. I used the 'tmpwatch' method. It hasn't actually run yet, but if it does everything it says it does, I'll be happy.
